This American study examines the associations of the caring role with lesbian, gay, bisexual, transgender, and queer (LGBTQ+) identity, social support, relationships, and care outcomes among LGBTQ+ older adults.

- Transgender and gender diverse adults were more likely to be caring for others living with mild cognitive impairment/Alzheimer's disease and related dementias (MCI/ADRD).
- LGBTQ+ MCI/ADRD carers primarily provide care to parents and familial relatives.
- MCI/ADRD care recipients hold differing political views than their LGBTQ+ carers.
- LGBTQ+ carers were less likely to have family, coworker, and neighbour support.
- LGBTQ+ carers were less likely to receive LGBTQ‐affirming care communication.

  • that is, those who are not connected biologically or legally. However, this study found that carers caring for someone living with MCI/ADRD were primarily familial relatives, demonstrating the need to further examine caring relationships with MCI/ADRD care recipients, how these relationships differ from chosen family networks and the impact on carers' health and relationships broadly.
    Understanding LGBTQ+ identity, social support and care access can inform targeted interventions to help reduce health disparities for LGBTQ+ carers.
