Long Summary

This review aimed to research whether integrated care improves outcomes for people living with long term neurological conditions. In addition, this article also seeks to clarify what characterises successful interventions. Overall, the evidence showed a mismatch between people's needs and outcomes measured, whereas successful programmes were characterised by expert knowledge, multisectoral care coordination, care continuity and a person-centred approach.

  • Integrated care had strong outcomes for decreased depression in patients, lesser burden on the carer and improved access to resources.
  • UK studies in particular indicated an overall improvement in patients' quality of life under integrated care.
  • This literature review shows that carers' needs remain unmet despite being under severe distress. It was often unclear on how carers' needs were identified and addressed.

  • Unmet needs of people living with long-term neurological diseases demonstrate how fragmented care delivery undermines the ability to meet their complex care needs.
  • Overall, policy suggests an integrated care response is needed.
  • Essential pillars that would result in improved outcomes are expert staff, good coordination between multisectoral providers, continuity of care and a person centred approach.
