The Carer Matters program was created to support family caregivers as they help their loved ones transition from hospital to home. Caregiving can be overwhelming, so this program provided personalised resources, tele-support, and training to ease the burden. Despite some initial challenges in reaching caregivers, the pilot enrolled 550 participants, many of whom found the support invaluable. Caregivers reported feeling less isolated and more confident in their roles, with 99% saying the training met their needs. Stakeholders saw the program as a meaningful way to bridge hospital care and home life, ensuring both caregivers and patients are well-supported.
